PROJECT-COURSE DESCRIPTION

In this project-course, participants will use digital health innovations (DHIs), more specifically, the GHF360 Comm.Health app suite (incorporating use of the GHF360 Personal.Health app), to facilitate development and augmentation of the capacity of community-/clinic-level health screening and vaccination initiatives to conduct service-/project-based systematic assessment, improvement & tracking of individual-level status, including group-level coverage (of health screenings and vaccinations).

WHAT YOU’LL LEARN

Participants who successfully complete the project-course will be able to undertake project-based use of the GHF360 Comm.Health app suite (incorporating use of the GHF360 Personal.Health app) to:

  • Register/create a GHF360 Comm.Health user account and Comm.Health project;
  • Login to GHF360 Personal.Health app as a Comm.Health app user and register/create a personal health app user/patient sub-account;
  • Use GHF360 Personal.Health App sub-accounts to conduct screenings and vaccinations status assessments (for each individual/patient derived from selected samples of service recipients in community/clinic settings);
  • Review GHF360 Personal.Health App-generated results (incl. PDFs) of individuals/patients’ screenings and vaccinations assessment status, follow-up interventions & tracking of individuals’ engagement progress;
  • Download/print & insert PDF of assessment results into patient medical record and use in follow-up care through community/clinic-level services;
  • Complete multiple screenings and vaccinations assessments in a population/selected sample of individuals/patients in a particular project setting (see sample project settings below);
  • Login to GHF360 Comm.Health app to create groups of selected individuals/patients, follow-up and track group-level (cascade of) progress across the continuum of engagement in prevention services on screenings and vaccinations;
  • Use app-generated analyses results to develop publishable reports summarizing project outcomes and demonstrate the utility of GHF360 DHIs;
  • Master the GHF360 App Suite interface and functionalities.

  • Public health agency, authority, or jurisdiction’s large-scale populations, incl. managed care health plans (or selected sub-population programs);
  • Workplace/occupational health sites;
  • Travel/transportation, hospitality & leisure services;

REQUIREMENTS

  • Access to reliable internet;
  • Laptop/tablet that can be used in field/project site;
  • Access to project setting(s) of interest;
  • Independent (or local academic-practice) partnership with a project host site preceptor;
